Joining up Self Care in the NHS
This initiative is evaluating the benefits of an integrated self care model in a pilot PCT and comprises of three major elements: disease prevention (coronary heart disease); a lay-led, condition-specific initiative for a long-term condition (adults with asthma); and minor-illness management by mothers and carers of young children.
Through the generation of self care behaviours and by influencing the actions taken by individuals, it is hoped that there will be a reduction in demand for GP consultations. Patients will become more familiar with all of the services that are available to them and, therefore, utilise alternatives to traditional general practice where appropriate.
WiPP formally completed its work on 30th June 2008. This site will remain live until March 2009.
